Insurance

We would be happy to file insurance for your eye exam if it is with a company that will work with us to do so. Currently, we can file for many plans under VSP, Spectera/United Healthcare, and Costco employees insurance. Unfortunately, we no longer can file for UAW.

VSP

In order for us to find your VSP benefits, we will need this information from you;

  • Primary insurer's legal name, date of birth, and the last 4 of their social security # (or unique ID).

  • If the exam is for a dependent, we also need their name and date of birth.

We can obtain your benefits thru Spectera if you provide us;

  • Unique Spectera/United Healthcare ID#, or

  • Last name and date of birth, or

  • Date of birth and last 4 of the SS#
